Internet Registration Fee/Entry Fee Billing: Entry fee schedule is a deferred payment plan. When a player registers, his/her credit card is charged a $5 nonrefundable registration fee that is paid to our online registration provider, Avance Connections. A "hold" will not be placed on the credit card for the tournament entry fee. After entries close, if a player is accepted into the tournament, a second charge for the entry fee amount is charged to the credit card. Your credit card statement will show charge(s) from Avanceconnections.com not Carolinas Golf Association.

By completing this online championship registration, you understand and agree that there is a $5.00 non-refundable internet registration fee that will be charged in addition to the championship entry fee whether you have been accepted, rejected or ineligible for this event. Any eligible refund will be credited to the credit card account submitted with this application minus the aforementioned internet registration fee.

Confirmation Number: At the end of the online registration process you may opt to receive an e-mail confirming the CGA's receipt of your entry. This confirmation number does not verify your acceptance into the championship, only that the CGA has received your application. The CGA will notify you of entry status.

Senior Four-Ball Series Tournament Regulations

2004 Senior Four-Ball Tournament Series changes are listed in BOLD type below.

Eligibility

Entry is open to any male amateur golfer who has reached his 55th birthday by the tournament date, is a legal resident of one of the Carolinas, is a member of a club which is a member of the Carolinas Golf Association, and has a current CGA handicap index (within 60 days of tournament entry opening date). A player with more than one CGA handicap index must use the lowest current index. Scores made in four-ball events must be posted to an individual's handicap as a tournament score. Entries are subject to rejection at any time including during the event, by the CGA or by the club hosting the tournament.

Rules

USGA Rules of Golf govern play. Local rules will be announced or posted at the starter's desk. The tournament will be 18 holes of four-ball stroke play, both gross and net. According to USGA handicap procedures, 90 percent of a player's course handicap will be used to determine net scores. Course handicaps are limited to 18 strokes.

If the difference between the partner's course handicaps exceeds eight strokes, then an additional 10 percent reduction will be applied to both team members. Teams are pre-flighted into two divisions according to total team course handicap. Partner substitutions made within five days of the tournament date must be approved in advance by the CGA office. Any on-site partner substitutions will result in the new partner playing at scratch and the team competing within the same flight.

Tournaments will be either double-tee or simultaneous (shotgun) starts. Competitors must subscribe to all host club restrictions concerning dress, coolers, etc. Cart rental is mandatory for tournament rounds. Practice rounds are not available. Metal and ceramic spikes are prohibited. Competitors may check tournament information at www.carolinasgolf.org. By submitting this application to the CGA, we agree to abide by all decisions of the CGA Tournament Committee, the CGA Tournament Manual (located at www.carolinasgolf.org) and the conditions of play pertaining to this tournament. We are aware that disqualification is the penalty for failure to do so. We release the Carolinas Golf Association, its committee members, officers, and staff from any and all liability for any event or consequence relating to entry or participation in this event.

Entry Information

Online entry applications may be completed at www.carolinasgolf.org. The $60 entry fee must accompany this form. Only one check per tournament entry. Make check payable to the Carolinas Golf Association. Applications are not accepted until 35 days (five weeks) prior to the event. Entries close 10 days prior to the event. Fax, e-mail, telephone, late or incomplete entries are not accepted. Entry forms will be accumulated for seven days after entry opening, at which time the field will be selected by the CGA. If the field is not filled at that time, entries will be accepted until the field is filled or the deadline is reached. Only eight teams from the host club will be accepted unless the field is not filled. Confirmation of acceptance will be sent to the applicant. A $20 administrative fee is deducted from all refunds. Refunds are not made after entries close except in the case of a verified player illness, injury or death in the family. Refunds are not made for any reason after five days prior to the tournament. When tournaments are scheduled on consecutive days, priority in acceptance is given to those who enter both events. Teams must submit a separate entry form for each tournament.

Double Tournaments

At select sites denoted on the tournament schedule, two separate tournaments (AM and PM) will be conducted at the same site on the same date. No special requests for AM or PM will be accepted.

Prizes and Merchandise

Merchandise will be awarded for low gross score in the "A" Division and for the low net scores in each flight. Merchandise certificates are redeemable only at the host club's golf shop. Ties are broken using the USGA recommended method.
