Eligibility: Entry is open to any team comprised of a male and a female (at birth) golfer who are members in good standing of a club which is a member of the Carolinas Golf Association and have current (within 90 days) CGA handicap indexes. Entries are subject to rejection at any time, including during the event, by the CGA or the host club. The reason for rejection may include unbecoming conduct.

Handicap Maximum: None

Format: The tournament will be contested at 36 holes. Teams will compete at four-ball stroke play using full course handicap. Lunch will be served to competitors followed by a shot-gun start each day. Each round will be a 1:00pm shotgun start.

Entries: Each team must complete an official entry form. No entry will be accepted prior to the opening date. Entries will be accepted until 5 p.m. of the closing date. The CGA is not responsible for errors or delays made by any mail or delivery courier. Fax, e-mail, telephone, late or incomplete entries are not accepted. Confirmation of acceptance will be mailed to the first player listed on the entry form. Pairings will be available to the host club and on the internet at www.carolinasgolf.org. Entries must be returned to the Carolinas Golf Association, PO Box 319, West End, NC 27376. The entry fee must be paid by each team and must accompany this form. Only one entry per check. Make check payable to Carolinas Golf Association. A check returned for insufficient funds may cause disqualification from future CGA competitions. A $10 administrative fee is deducted from all refunds. Refunds are not made for any reason after entries close.

Internet Registration Fee/Entry Fee Billing: Entry fee schedule is a deferred payment plan. When a player registers, his/her credit card is charged a $5 nonrefundable registration fee that is paid to our online registration provider, Avance Connections. A "hold" will not be placed on the credit card for the tournament entry fee. After entries close, if a player is accepted into the tournament, a second charge for the entry fee amount is charged to the credit card. Your credit card statement will show charge(s) from AVANCECONNECTIONS.COM from New Jersey not Carolinas Golf Association. Do not issue a chargeback to your credit card company. Chargebacks will be assessed a $20 service fee in additional to any entry and registration fees.

By completing this online tournament registration, you understand and agree that there is a $5.00 non-refundable internet registration fee that will be charged in addition to the championship entry fee whether you have been accepted, rejected or ineligible for this event. Any eligible refund will be credited to the credit card account submitted with this application minus the aforementioned internet registration fee.

Confirmation Number: At the end of the online registration process you may opt to receive an e-mail confirming the CGA's receipt of your entry. This confirmation number does not verify your acceptance into the championship, only that the CGA has received your application.

Entry Fee: $80 plus $5 internet convenience fee.
